I had a total of 50 practicum hours at Cedars-Sinai Medical Center, outpatient in the internal medicine department. This clinical site provided exposure to a wide range of adult patients with chronic and complex health conditions. The practicum hours were all completed and were non-compensated and were not part of employment obligations under the direct supervision of a qualified preceptor.
The practicum experience involved me administering thorough patient assessments, such as health history, medication, and physical examinations. I helped in the management of chronic conditions like coronary artery disease, chronic kidney disease, diabetes mellitus, chronic obstructive pulmonary disease (COPD), and major depressive disorder. One of the areas of my practicum was patient education. I also offered guidance on lifestyle change, such as dietary changes, physical exercise prescriptions, coping with stress, and quitting smoking. I have also strengthened medication compliance measures and described the possible side effects to enhance patient knowledge and safety. These interactions increased my communication skills and capacity to package information according to different levels of health literacy.
I was also involved in interdisciplinary team discussions with the physicians, nurse practitioners, pharmacists, and case managers. The focus of these meetings was to achieve collaborative practice and multifaceted health concern patient care planning. The reflective feedback stations with my preceptor enabled me to review my thinking in clinical practice, perfect my assessment skills, and define my professional development areas.
Altogether, the experience in this internal medicine environment, comprising 50 structured practicum hours, contributed to enhancing my competence in advanced assessment, chronic disease management, evidence-based treatment planning, and collaboration with other professionals. The experience increased my confidence and readiness to work in the advanced nursing practice and strengthened the principles of patient-centered and quality-intensive care.
